CCP Certification
CCP CertificationOffered in partnership with DMAA, CCP certification is the only nationally recognized, award-winning learning program in health coaching, disease management and chronic care management. CCP is now required or recommended by leading purchasers, health plans, provider groups, and military and state health care programs. This validated program prepares clinicians to deliver effective, evidence-based patient engagement, activation, self-care, adherence and lifestyle management interventions.

This 40-hour, competency-based blended learning program combines a live learning session, comprehensive online curriculum, and the largest motivational interviewing (MI) video library. CCP includes key content on the foundations of population health improvement, evidence-based medical care and wellness guidelines, and brief MI techniques. Attendees must also complete the online CCP eLearning program and online certification examination within one year of registration. Organizations and programs that offer CCP certification to their clinical staff may also be eligible for CCP staff accreditation—documenting preparation in evidence-based population health improvement foundations, approaches and interventions.
