Through the 2010 Quality Impact Awards program, DMAA: The Care Continuum Alliance seeks to promote the contributions of population-based interventions to health care quality. The Quality Impact Awards program is part of a broader quality initiative spearheaded by the DMAA: The Care Continuum Alliance Quality Improvement Steering Committee that also includes an online Quality Improvement Case Studies Registry and collaborations with other health care quality organizations.Please note: The 2010 Leadership and Quality Impact awards nomination process is now closed.

- Greatest Impact on Health
- Greatest Impact on Satisfaction
- Most Innovative Program Design or Delivery
- Outstanding Physician
Quality Impact Award for Greatest Impact on HealthThis award recognizes a company, provider group or government initiative that has implemented a quality population health management program with proven impact on the health of the target population. Examples may include, but are not limited to: improved overall health, decrease in number of risk factors, whole person orientation, patient safety and quality, coordinated and integrated care delivery, and continual improvement of care and services in population health management. Of particular interest will be programs that demonstrate outcomes along the full continuum of population health management.
As well as core criteria, judges will look for evidence of:
- Outcomes or measurement data including a detailed statistical analysis of the outcomes.
- Adequate population size to demonstrate statistical significance.
- Evidence of sustainability—the results must be demonstrated over sufficient time (minimum: 18 months) to demonstrate sustainability.
- Transferrability and how the program might be implemented or adapted across the industry.
- A detailed explanation of the methodology.
- A clear description of the intervention and its impact on health.
Quality Impact Award for Greatest Impact on SatisfactionThis award recognizes a company, provider group or government initiative that has implemented a quality population health management program with the greatest impact on satisfaction of a target population of participants/members or providers.
As well as core criteria, judges will look for evidence of:
- Successful approaches to participant or provider engagement.
- Increased rates of participation over time.
- Enhanced access and participant or provider communication.
- Increased satisfaction related to successful approaches (e.g., participation, communication, improved health).
- Evidence of sustainability—the results must be demonstrated over sufficient time (minimum: 18 months) to demonstrate sustainability.
- Outcomes that used a satisfaction tool for measurement (e.g. DMAA: The Care Continuum Alliance Participant Satisfaction Survey or Provider Satisfaction Survey, or other satisfaction measurement tool).
Quality Impact Award for Most Innovative Program Design or DeliveryThis award recognizes a person, team or company which has created new and innovative methods in one of two areas.
• Design process or delivery. Examples may include, but are not limited to: initiatives for conditions not managed by other organizations; integrated approaches to multiple condition management; new methods for delivering information or communicating with participants; collaborative programs between organizations; and integration with an existing program.
• Staff recruitment, development and management. Examples may include, but are not limited to: innovative hiring techniques and approaches; innovations in the ability to deliver services, to be more effective with participants; and improved efficiency.
As well as core criteria, judges will look for evidence of:
- Evidence of innovation (e.g. literature references) and how the program innovation is compared to other standard approaches.
- Innovations that are sustainable examples for improving health outcomes, support and independence.
- Indication of how outcomes were impacted.
- Proof of quality improvement tools or approaches.
- Transferrability and how the program might be implemented or adapted across the industry.
- A detailed explanation of the methodology.
Quality Impact Award for Outstanding PhysicianThis award recognizes a physician who has demonstrated a commitment to quality in chronic care management and coordination. As well as core criteria, judges will look for evidence of:
- proven commitment to quality and influencing the adoption of best practices by peers. Include description of the best practice from a quality perspective.
- Integration, including the fostering of positive relationships that raised awareness of the value of chronic care management and built trust between the health care community and patients.
- Innovation, through program implementation or physician engagement.
