Recognizing Exceptional Care Quality in Population Health Improvement
Since 1999, DMAA: The Care Continuum Alliance Leadership Awards have honored organizations and individuals in population health management. Although the awards have evolved over the years, their core purpose remains the same: to recognize outstanding achievements in and contributions to the science and practice of wellness, prevention and chronic care management.In 2008, DMAA: The Care Continuum Alliance expanded its awards program to include annual Quality Impact Awards, which support the association's dedication to highlighting quality efforts in health care reform, health promotion and health care delivery.
