DMAA Provider Satisfaction Survey—User Agreement

DMAA: The Care Continuum Alliance (DMAA) has created the Provider Satisfaction Survey for use by employers, health plans and other organizations interested in assessing provider attitudes toward disease management and other population-based programs.

Completion of this user agreement is required for access to and use of the survey tool. Once the user agreement is complete, the download will be available.

The purpose of this user agreement is to allow DMAA to track survey use and ensure users fully understand the survey's benefits. DMAA will not share this information with third parties or use it for commercial purposes.
