* To qualify for this category, the registrant must be employed full-time by a federal, state or local government (contractors do not qualify); a full-time employee or student at a college or university and not employed by any health care organization; or work and live outside the United States.
Employers receive a 20 percent discount off non-member registration fees for the Integrated Care Summit. Simply enter this special promotional code, "EMPL", on the hard-copy registration form and fax or mail it back to DMAA. To qualify for the employer rate, a registrant must be an executive responsible for purchasing and administering health benefits within an organization that is not in the business of providing health care products. Example of titles in this category include: vice president of human resources, director of benefits, benefits manager and chief medical officer. Documentation to verify eligibility may be required to process registration discounts. Note: This discount may not be combined with online registration, member or group discounts.
DMAA is pleased to offer a guest pass designed to provide attendees' spouses, significant others and adult children (at least 21 years of age) access to the Exhibit Hall for evening receptions for $25 per reception. A guest pass may be purchased only at onsite Registration by the attendee who is registered for The Forum 08 or Integrated Care Summit. Employees of health care related companies are ineligible for guest passes. A guest pass does not include conference materials, luncheons or provide access to educational sessions.
