Become a Member and Save on Registration

Three Ways to Register
DMAA offers three convenient registration options for the Integrated Care Summit:
  1. Online
    Secure Internet booking with confirmation is available 24 hours a day, seven days a week. Credit card only. NAM, CMSA, AAPPO Members: You must register by fax or mail (below) to receive the DMAA member rate. Go to online registration»
  2. Fax (Use for NAM, CMSA, AAPPO discounts)
    Download, print and complete a paper registration form and send by fax to (202) 737-5738. Credit card only. Allow five business days for processing and receipt of registration confirmation.
  3. Mail (Use for NAM, CMSA, AAPPO discounts)
    Download, print and complete a paper registration form and submit by postal mail to:

    Credit card or check (drawn on U.S. bank in U.S. funds). No wire transfers or purchase orders will be accepted. Allow five business days for processing and receipt of registration confirmation.
