Three Ways to Register
DMAA offers three convenient registration options for the Integrated Care Summit:
  1. Online
    Secure Internet booking with confirmation is available 24 hours a day, seven days a week. Credit card only. NAM, CMSA, AAPPO Members: You must register by fax or mail (below) to receive the DMAA member rate. Go to online registration»
  2. Fax (Use for NAM, CMSA, AAPPO discounts)
    Download, print and complete a paper registration form and send by fax to (202) 737-5738. Credit card only. Allow five business days for processing and receipt of registration confirmation.
  3. Mail (Use for NAM, CMSA, AAPPO discounts)
    Download, print and complete a paper registration form and submit by postal mail to:
