Those who register online for The Forum 08 will automatically save $50! This discount cannot be used for special rates, symposiums, the Integrated Care Summit or group rate discounts. Online discounts will not be accepted for mailed or fax registrations.

NAM, CMSA, AAPPO, SOA & NACDD Members: Enjoy Preferred Pricing

DMAA has partnered with the National Association of Manufacturers (NAM), the Case Management Society of America (CMSA), the American Association of Preferred Provider Organizations (AAPPO) and the Society of Actuaries (SOA) to offer their members discounted DMAA member registration rates for The Forum 08.

Members of the National Association of Chronic Disease Directors (NACDD) will receive a special rate of $500 to The Forum 08.

To receive these discounts, you must submit your registration form by fax, (202) 478-5113; or by postal mail, to DMAA: The Care Continuum Alliance, 701 Pennsylvania Ave. NW, Suite 700, Washington, D.C. 20004. Online registrations, group discounts and special rates do not qualify for this discount. DMAA will verify your membership status and process your registration within five (5) business days.

Group Discounts for DMAA Members

For every two paid attendees from your organization who register for The Forum 08, a third employee receives a free Forum registration. Groups must submit their registration forms by mail or fax and pay together to receive the savings. Fax team registrations to (202) 478-5113. Registrations for symposiums, special rates, the Integrated Care Summit and day passes are excluded are excluded from this promotion.

Group Discount Restrictions:

Group discounts are available only to DMAA members.
Each group registrant must complete a registration form.
The three forms must be received together with complete payment information to be eligible for the promotion.
Group discounts are available only for The Forum 08 registrations. Registrations for symposiums, special rates, the Integrated Care Summit and day passes are excluded from this promotion.
Group discounts are not available for online registrations.
If, due to cancellations, a group falls to fewer than three registrations, the group discount will be forfeited and the standard registration fees will be assessed. Registrations not meeting these requirements will be responsible to pay any balances due at onsite registration.
