Chronic Care Professional (CCP) Certification Program
Chronic Care Professional CertificationPopulation health improvement is a new model of care that requires new competencies. CCP Certification is the only nationally recognized population health improvement professional certification. A growing number of health plans and disease management organizations are requiring or recommending this highly acclaimed certification program, now offered in partnership with DMAA.
This program features an advanced blended learning approach. The live session combines core program content with active learning to build critical patient partnering, behavior change and health coaching competencies. Following completion of the live program, attendees complete the self-directed CCP eLearning program and online certification examination, on their own, outside the session. The entire blended learning program is 40 hours long.

Continuing Medical Education Credits
This program is not eligible for continuing medical education credits through DMAA. Through the HealthSciences Institute, the continuing nursing education activity was approved by the Wisconsin Nurses Association Continuing Education Approval Program Committee, an accredited approver by the America Nurses Credentialing Center's Commission on Accreditation. The program has been approved for 40 contact hours for nurses.
