DMAA Population Health Improvement Leadership Awards
Since its creation in 1999, DMAA's Leadership Awards Program has honored organizations and individuals in population health improvement annually. Although the awards have evolved over the years, their core purpose remains the same: to recognize outstanding achievements in and contributions to the science and practice of population-based care.The submission period for 2008 Leadership Awards has closed. 2008 Population Health Improvement Leadership Awards:
Award submissions must include the following specifications to be considered for presentation:
- Complete contact information (including phone, fax, e-mail and postal address) for both the sponsor and nominee.
- An award summary, not exceeding 300 words, appropriate for inclusion in the onsite Program Guide and Web site.
- A brief description, 150 words or fewer, on how the submission meets each of the core criteria: innovation, commitment, outcomes-based orientation, scope and potential impact, program design and implementation, and vision and leadership.
- A detailed description, not exceeding 300 words, on how the submission meets all the category-specific criteria.
All applications for Population Health Improvement Leadership Awards will comply with the core criteria, as well as the category-specific criteria, relevant to each of the applications.
Core criteria for judging:
- Commitment—Exhibits measurable organizational commitment to population-based care.
- Program Design—Successfully employs science and evidence-based practice guidelines in the design of population health improvement programs.
- Implementation—Establishes excellence in the extended execution of population health improvement programs.
- Innovation—Demonstrates improvements to the science of population-based care by applying innovative strategies that overcome challenges and barriers.
- Outcomes-based Orientation—Provides robust, objective evidence of financial and health outcomes.
- Scope and Potential Impact—Degree to which the success of the initiative and effort could potentially impact the population health improvement community and change the lives of those served by it.
- Vision and Leadership—Demonstrates commitment, vision and leadership to the principles of population health improvement.
Comprehensive Corporate Leadership
This award recognizes an organization that offers a comprehensive platform of integrated health management tools, services and programs to assist in managing patient populations. As well as core criteria, judges will look for evidence of:
- A unique level of leadership and innovation in the industry.
- Integration of all health management areas—wellness and prevention care, and management of acute events and chronic illnesses.
- High-quality standards for key components of wellness, disease, case management and care coordination programs, as well as support services and materials.
Outstanding in Emerging Health Needs
This award recognizes a company that has demonstrated a commitment to an innovative use of new programs to address issues of behavior change, racial disparities or cultural competency in its population health improvement program. As well as core criteria, judges will look for evidence of:
- A commitment to, vision for and leadership of creative application of such innovative programs (behavior change, racial disparities or cultural competency) in population health improvement.
- Innovation in the design and/or implementation of population health improvement programs.
- A thoughtful approach to measuring the impact of intervention on the overall program performance.
- Identification and implementation of best practices for care delivery and reimbursement models.
Outstanding Employer Program
This award recognizes an employer organization that has demonstrated leadership in the design and implementation of population health improvement programs for its employees. As well as core criteria, judges will look for evidence of:
- Program innovation that successfully integrates population-based care into employee benefit schemes and productivity management.
- Success of the program validated by carefully designed methodology as evidenced by response rate of employees, favorable outcomes, measurable return on investments and increased productivity.
- Proven commitment to wellness, prevention and population health improvement programs that promote healthful lifestyle choices and activities.
Outstanding Government Program
This award is designed to recognize an organization that has implemented a population health improvement program for the Medicare or Medicaid population. As well as core criteria, judges will look for evidence of:
- Excellence in the design, development, implementation and operation of a population health improvement program resulting in favorable, measurable outcomes.
- A unique level of leadership and innovation in the industry.
- Advocacy of the principles and benefits of population health improvement to public health officials, including state and federal government entities.
Outstanding Health Plan
This award recognizes a health plan or managed care organization that has successfully implemented a population health improvement program in its market. As well as core criteria, judges will look for evidence of:
- Excellence in the design, development, implementation and operation of a population health improvement program with success demonstrated by favorable, validated outcomes.
- A unique level of leadership and innovation in the industry.
- Support of delivery system models that ensure appropriate care for chronic conditions and coordination among all health care providers.
Outstanding Individual
This award recognizes individuals within the population health improvement community who have consistently helped advance population-based care. As well as core criteria, judges will look for evidence of:
- A proven history of commitment to shaping and advancing population health improvement over time, transforming research and promoting pro-active, patient-centric focus across the continuum.
- Leadership, including the fostering of positive relationships that raised awareness of the value of population health improvement, built trust between the health care community, consumers, and payers—both public and private.
- Development of results-oriented policies or programs that enhance various aspects of population health improvement quality and delivery of care.
Outstanding Journal Article
This award recognizes excellence in published articles on population health improvement. A PDF file of the full text of the article must be included with the submission form. As well as core criteria, judges will look for evidence of:
- Publication in a peer-reviewed journal.
- The degree to which the article introduces concepts of an original nature, including original research.
- The quality of the writing, degree of referencing and success in achieving the objectives of the article.
Outstanding Provider Engagement Initiative
This award recognizes an organization for initiatives that successfully engaged treating physicians and increased physician support of a population health improvement program. As well as core criteria, judges will look for evidence of:
- Positive relationships with treating physicians that are proved to raise awareness of the value of population-based care.
- Increased physician commitment and support of the program.
- Convening health care professionals across the care continuum to share and integrate practice models.
Outstanding Quality Initiative
This award will go to the organization that demonstrates quality care in terms of measurable outcomes with a systematic approach to population health improvement. As well as core criteria, judges will look for evidence of:
- Program incorporates evidence-based outcomes measures that demonstrate improvements in health, satisfaction or financial outcomes achieved through wellness, disease and/or case management care coordination programs.
- Program demonstrates an evaluation process that shows before and after changes as a result of program implementation.
Outstanding Technology/Innovative Tool
This award will go to a health care organization that demonstrates an innovative model or best use of technology toward achieving and maintaining improved health outcomes. As well as core criteria, judges will look for evidence of:
- Innovation in the design and implementation of technology/enabling tools.
- Enhanced performance following implementation validated by carefully designed methodology.
- Expansion beyond traditional voice and data services to offer a full array of innovative solutions.
- Interoperability of health information technologies.
Physician in Practice
This award recognizes physicians in individual or group practice who have consistently helped advance chronic care management and coordination. As well as core criteria, judges will look for evidence of:
- A proven history of commitment to advancing best and evidence-based practices in the awardee's practice and influencing the adoption of best practices by peers.
- Integration, including the fostering of positive relationships that raised awareness of the value of chronic care management and built trust between the health care community and patients.
- All award submissions must be submitted electronically by 8 p.m. Eastern Time, Friday, June 13, 2008.
- We welcome awards submissions in the form of nominations, personal applications or applications on behalf of a team.
- Some of the work involved in your award submission must have been undertaken in 2007/08.
- Submissions with system status of "incomplete" cannot be processed.
- There is a limit of one abstract per sponsor that can be submitted.
- You do not need to be a DMAA member to submit an award submission.
- You may submit a piece of work previously entered for the DMAA Awards Program only if sufficient progress has been made in 2007/08 to merit renewed consideration.
- Award submissions containing identical or nearly identical information submitted from the same institution and/or individuals will be disqualified.
- Award submissions for the Outstanding Journal Article must include PDF files of the actual articles as part of the submissions to be considered.
- Proofread award submissions to avoid errors before submission.
- Award judges may re-categorize an entry if a determination is made that an entry submitted for one category is of greater relevance to another category.
- DMAA reserves the right to not present awards if the category submissions do not meet 75 percent of the core criteria.
- DMAA may edit nominations to correct any erroneous administrative information.
- DMAA cannot provide feedback on individual award nominations.
