DMAA Population Health Improvement Leadership AwardsSince its creation in 1999, DMAA's Leadership Awards Program has honored organizations and individuals in population health improvement annually. Although the awards have evolved over the years, their core purpose remains the same: to recognize outstanding achievements in and contributions to the science and practice of population-based care.

Core Criteria
All applications for Population Health Improvement Leadership Awards will comply with the core criteria, as well as the category-specific criteria, relevant to each of the applications.

Comprehensive Corporate Leadership
This award recognizes an organization that offers a comprehensive platform of integrated health management tools, services and programs to assist in managing patient populations. As well as core criteria, judges will look for evidence of:
Outstanding in Emerging Health Needs
This award recognizes a company that has demonstrated a commitment to an innovative use of new programs to address issues of behavior change, racial disparities or cultural competency in its population health improvement program. As well as core criteria, judges will look for evidence of:
Outstanding Employer Program
This award recognizes an employer organization that has demonstrated leadership in the design and implementation of population health improvement programs for its employees. As well as core criteria, judges will look for evidence of:
Outstanding Government Program
This award is designed to recognize an organization that has implemented a population health improvement program for the Medicare or Medicaid population. As well as core criteria, judges will look for evidence of:
Outstanding Health Plan
This award recognizes a health plan or managed care organization that has successfully implemented a population health improvement program in its market. As well as core criteria, judges will look for evidence of:
Outstanding Individual
This award recognizes individuals within the population health improvement community who have consistently helped advance population-based care. As well as core criteria, judges will look for evidence of:
Outstanding Journal Article
This award recognizes excellence in published articles on population health improvement. A PDF file of the full text of the article must be included with the submission form. As well as core criteria, judges will look for evidence of:
Outstanding Provider Engagement Initiative
This award recognizes an organization for initiatives that successfully engaged treating physicians and increased physician support of a population health improvement program. As well as core criteria, judges will look for evidence of:
Outstanding Quality Initiative
This award will go to the organization that demonstrates quality care in terms of measurable outcomes with a systematic approach to population health improvement. As well as core criteria, judges will look for evidence of:
Outstanding Technology/Innovative Tool
This award will go to a health care organization that demonstrates an innovative model or best use of technology toward achieving and maintaining improved health outcomes. As well as core criteria, judges will look for evidence of:
Physician in Practice
This award recognizes physicians in individual or group practice who have consistently helped advance chronic care management and coordination. As well as core criteria, judges will look for evidence of:
Policies
  1. All award submissions must be submitted electronically by 8 p.m. Eastern Time, Friday, June 13, 2008.
  2. We welcome awards submissions in the form of nominations, personal applications or applications on behalf of a team.
  3. Some of the work involved in your award submission must have been undertaken in 2007/08.
  4. Submissions with system status of "incomplete" cannot be processed.
  5. There is a limit of one abstract per sponsor that can be submitted.
  6. You do not need to be a DMAA member to submit an award submission.
  7. You may submit a piece of work previously entered for the DMAA Awards Program only if sufficient progress has been made in 2007/08 to merit renewed consideration.
  8. Award submissions containing identical or nearly identical information submitted from the same institution and/or individuals will be disqualified.
  9. Award submissions for the Outstanding Journal Article must include PDF files of the actual articles as part of the submissions to be considered.
  10. Proofread award submissions to avoid errors before submission.
  11. Award judges may re-categorize an entry if a determination is made that an entry submitted for one category is of greater relevance to another category.
  12. DMAA reserves the right to not present awards if the category submissions do not meet 75 percent of the core criteria.
  13. DMAA may edit nominations to correct any erroneous administrative information.
  14. DMAA cannot provide feedback on individual award nominations.
